Mary D. Naylor

Turn-Key Health (TKH), an Enclara Healthcare company, serving health plans, provider organizations and their members who experience a serious or advanced illness, announces that Mary D. Naylor, Ph.D., RN, FAAN, the Marian S. Ware Professor in Gerontology and director of the New Courtland Center for Transitions and Health at the University of Pennsylvania School of Nursing, has joined its advisory board.

Dr. Naylor brings more than two decades of experience leading a multidisciplinary team of clinical scholars and health services researchers in generating and disseminating research findings designed to enhance the care and outcomes of chronically ill older adults and their family caregivers.

The architect of the Transitional Care Model, she and her team designed a care management approach proven in multiple NIH clinical trials and foundation-sponsored implementation efforts to improve older adults’ experience with care and health outcomes, while decreasing use of costly health services.

Currently, Dr. Naylor is a member of the National Academy of Medicine’s Leadership Consortium on Value & Science-Driven Health Care and co-chairs the Care Culture and Decision-Making Innovation Collaborative. She is also a member of the RAND Health Board of Advisors, the Agency for Healthcare Research and Quality’s National Advisory Council and the Board of the Dorothy Rider Pool Health Care Trust. Additionally, she serves as a member of Villanova University’s Board of Trustees. Dr. Naylor received her Bachelor of Science in Nursing from Villanova University, her Master of Science in Nursing and Ph.D. from the University of Pennsylvania.
